Why do I need to complete the work history report?

One of the steps in establishing disability is proving that your medical condition prohibits you from doing the kind of jobs that you have worked in the last 5 years. Social Security will send you a Work History Report (Form SSA-3369) to collect information about the jobs that you have worked over the most recent 5 years. You will have to list all of the jobs that you have had in the last 5 years. You will need to provide a detailed summary of the work you did. You will need to describe how much the job required you to walk, stand, sit, handle big objects, reach, write, type, handle small objects, and other physical demands. You will also have to describe the mental demands of the job and whether the job required special skills or training/certification. If you were a lead worker or supervised other employees, you should include that information. It is important to provide as much information as possible about each job because Social Security will use this report to decide whether or not you will be able to do any of those jobs again now with the impairments that you have due to your medical condition.
