The burden of proof is on you, the claimant, to prove you are disabled. One of the ways we do that is through medical records. As your attorney, we request medical records on your behalf and submit them to the Social Security Administration. In order to request your medical records we need two things: 1. A list of all the doctors you have seen and where they are located, we cannot know this information without you telling us; and 2. A signed release of information form which we submit to the provider allowing us to request the records. Without these things, we cannot request medical records on your behalf, so it is imperative that you communicate with us regularly to let us know the doctors you are seeing.
