Alright already!
As if being sick and tired and feeling not yourself all the time, now you have to fill out pages and pages of information for Social Security…once, twice….maybe even three times?? What gives?
Functional reports, work history reports, release of information forms…these are just some of the many forms you will encounter during your disability application and throughout the life of your claim. It can be hard completing these when you are tired or confused or your hands are in pain from arthritis.
Keep in mind how important these forms are though! Filling them out with detailed, honest, accurate information can mean the difference between getting denied over and over and getting that much needed benefit award!
The good news is that you CAN ask for help in completing these forms. If you have a family member, friend, social worker, nurse, or other helpful person in your life, and they are available, they can write or type the information for you. You should still answer all the questions in your own words. Or you can work on it a little every day. Sometimes your lawyer’s office can assist as well. Simply call to make an appointment!
