Why do I need to complete the Function Report?

Social Security will consider how your disabilities affect your daily life. After you apply for disability, Social Security will send you a Function Report (Form SSA-3373) to collect information about your ability to perform activities of daily living, such as cooking, cleaning, bathing, getting dressed, taking care of children or pets, and using the bathroom. It is important to provide as much detail as possible because Social Security staff use the Function Report to determine if your medical condition is severe enough to impact your daily functioning and to see if your statements are consistent with your medical records. If you fail to complete the form, Social Security might deny you due to having insufficient evidence to evaluate your claim.
